I do hereby voluntarily submit my application for attendance and participation in Bright Brazilian Jiu-Jitsu classes or events.
As a member of Bright Brazilian Jiu-Jitsu, I hereby assume full responsibility for any and all damages, injuries or loss that I may sustain or incurred while attending or participating in any event or class with Bright Brazilian Jiu-Jitsu.
As a member of Bright Brazilian Jiu-Jitsu, I hereby waive all claims against the building owners, instructors, representatives, volunteers, participating students or owners of Bright Brazilian Jiu-Jitsu.
I give permission to Bright Brazilian Jiu-Jitsu to use photos, videos of me and/or my child for marketing purposes (Facebook posts, Website, etc.). Bright Brazilian Jiu-Jitsu will NOT misuse photos and videos. I understand I have the right to request removal of any photos and videos of me and/or my child.
I hereby understand that Covid/Pandemic cases may result in a shutdown, whether governmental shutdown or a building shutdown. I understand that Bright Brazilian Jiu-Jitsu is not responsible for any class time lost and will NOT be issuing refunds.
